IMPORTANT:  In order to expedite your annuity application with any company, be sure to have all of the necessary paperwork required and completed before sending to the home office.  This will ensure that your business is processed as quickly as possible.  Usually, most companies require a maximum of 5 forms:

1. New Business Transmittal Letter
2. Application For Single Premium And Flexible Premium
3. Product Disclosure / Summary
4. State Disclosure (Example:  CA and OH)
5. W-9
6. Transfer Form (if transferring funds)
7. Replacement Form (if transferring funds)
