IMPORTANT:  In order to expedite your annuity application with any company, be sure to have all of the necessary paperwork required and completed before sending to the home office.  This will ensure that your business is processed as quickly as possible.  Usually, most companies require a maximum of 5 forms:

1. Annuity Application
2. Specific Product Disclosure
3. Over Age 65 Consumer Disclosure
4. Transfer Request or 1035 Exchange Form (if transferring funds)
5. Specific State Replacement Form (if transferring funds)
