IMPORTANT: In order to expedite your annuity application with any company, be sure to have all of the necessary paperwork required and completed before sending to the home office. This will ensure that your business is processed as quickly as possible. Usually, most companies require a maximum of 5 forms:
Order Ticket Form (request/application form)
CLU Form (Client Letter of Understanding)
AML Form (notice and customer information form)
Transfer Form (if transferring funds)
Replacement Form (if transferring funds)
